1. Graduate from a JRCert accredited program (certificate or associate) in radiologic technology.
2. Be ARRT or eligible for ARRT certification
3. Pre-requisites: English Composition I & II to be taken before DENG 315 Technical writing (see Transferring Credits)
4. Student must complete a minimum of 6 additional hours of lower division credit (with a grade of C [2.0] or higher) with courses to be chosen, with the approval of the advisor, from behavioral sciences, mathematics - computer sciences and / or social sciences.(see Transferring Credits)

(Please use one form for each course transfer request)
Use this form together with an official transcript to request credit transfer from a regionally accredited college or professional program. Only courses that are required in the curriculum will be transferred. For courses that are not clearly defined you may be asked to submit course descriptions and syllabi for further evaluation. Use one form for each course transfer request.

Enrollment Verification Form
Use this form to request formal verification of your enrollment in the degree program to submit to your employer, VA or other institution requesting such information.

Withdrawal Form
Use this form to officially withdraw from the course of program.
